“Pulled Elbow” in Children

A practical, safe and low-resource approach to diagnosis, reduction, post-reduction assessment and parent advice.

Clinical overview

Radial head subluxation, often called pulled elbow or nursemaid’s elbow, is a common injury in children, classically between 1 and 4 years of age. The diagnosis is clinical when the history and examination are typical: sudden refusal to use the arm, the elbow held extended or slightly flexed, forearm pronated, and no swelling, bruising, deformity or focal bony tenderness.

Typical mechanism

Axial traction on the child’s hand or forearm, often while the forearm is pronated. A clear pulling history may be absent.

Key pathology

The annular ligament slips proximally and becomes interposed between the radial head and capitellum.

When X-ray is needed

Plain radiographs are not routinely required when the presentation is classic and there are no red flags. Image before repeated manipulation if the diagnosis is uncertain.

  • Swelling, bruising, deformity, warmth, erythema or significant focal tenderness.
  • Fall from height, direct trauma, unwitnessed injury with concerning history, or safeguarding concern.
  • Atypical age, especially very young infant or older child with a non-classic presentation.
  • Failed reduction after two well-performed attempts, or persistent non-use of the limb after observation.

Reduction technique

Explain the manoeuvre to the parent. The child may cry during reduction, but pain relief is usually rapid once reduction succeeds. Sedation is not required in a typical case.

Analgesia, when needed: if presentation is delayed or the diagnosis is unclear, use simple analgesia before reduction. Paracetamol 10–15 mg/kg PO every 6 hours as needed, maximum 60 mg/kg/day. Ibuprofen 10 mg/kg PO every 6–8 hours as needed, maximum 30 mg/kg/day; avoid in dehydration, renal impairment, active gastrointestinal bleeding/ulcer disease, NSAID hypersensitivity, and infants under 3 months unless local protocol permits.
Preferred first attempt

Hyperpronation

Seat the child on the parent’s lap. Support the elbow with one hand and place the thumb over the radial head. With the other hand, hold the distal forearm or wrist and smoothly pronate the forearm fully. A subtle click may be felt over the radial head.

Alternative technique

Supination–flexion

Support the elbow and palpate the radial head. Supinate the forearm fully, then flex the elbow completely in one controlled movement. This is useful if hyperpronation fails or if the clinician is more experienced with this method.

After the manoeuvre

Observe function

Review the child after 10–15 minutes. Offer a toy, drink, sticker or phone above shoulder level and watch for spontaneous reaching. Successful reduction usually restores painless arm use within 5–30 minutes.

Reduction should be smooth and controlled. Forceful traction is unnecessary and may increase pain.

If reduction fails

  • Wait 10–15 minutes and reassess; sometimes function returns gradually.
  • Repeat once using the alternative technique.
  • If the child still refuses to use the arm after two attempts, obtain X-rays and reassess the diagnosis.
  • Consult orthopaedics/paediatrics if imaging is abnormal, the diagnosis remains uncertain, or reduction cannot be achieved.
Pitfall: do not keep repeating painful manipulations in a swollen or tender elbow. Treat that presentation as a possible fracture until proven otherwise.

Post-reduction care

No cast, splint or sling is needed after a clearly successful reduction. Discharge is appropriate once the child is comfortable and using the limb normally.

Stage Expected finding Action
10–15 min Less pain; early spontaneous movement. Observe, encourage reaching and play.
5–30 min Normal or near-normal arm use in most acute cases. Discharge with parent advice if function returns.
>12 h old injury Arm use may take 1–2 days even after successful reduction. Broad arm sling for comfort and review in 48 h if not improving.
Persistent non-use No return of function after appropriate observation or repeat technique. X-ray, reconsider fracture/dislocation/infection, escalate.

Parent advice

  • Do not lift, swing or pull the child by the hand, wrist or forearm.
  • Lift the child from under the arms or around the trunk.
  • Return urgently if swelling, bruising, fever, increasing pain, deformity, or persistent refusal to use the arm develops.

Low-resource emergency approach

When radiography is limited, the safest strategy is strict clinical selection: reduce only the classic painless-looking, non-swollen pulled elbow. Any swelling, bruising, focal tenderness, deformity, systemic illness or uncertain mechanism should be immobilised as a possible fracture and referred for imaging when available.

Підвивих головки променевої кістки у дітей

Підвивих головки променевої кістки, або pulled elbow, — типова травма дітей раннього віку, найчастіше 1–4 років. Діаґноз зазвичай клінічний: дитина раптово перестає користуватися рукою, тримає лікоть розігнутим або злегка зігнутим, передпліччя — у пронації, без набряку, синців, деформації чи чіткої кісткової болючости.

Типовий механізм

Різкий потяг за кисть або передпліччя, часто при пронації. Водночас очевидної історії “потягнули за руку” може не бути.

Суть ушкодження

Кільцева зв’язка зміщується проксимально й інтерпонується між головкою променевої кістки та capitellum.

Коли потрібна рентґеноґрама

При класичній картині рентґеноґрама не є рутинно потрібною. Знімок необхідний, якщо є ознаки іншого ушкодження або вправлення неуспішне.

  • Набряк, синець, деформація, локальна кісткова болючість, гіперемія або підвищення температури шкіри.
  • Падіння, прямий удар, неясний механізм або підозра на неакцидентальну травму.
  • Нетиповий вік або нетипова клінічна картина.
  • Відсутність ефекту після двох технічно правильних спроб вправлення.

Техніка вправлення

Поясніть маневр батькам. Дитина може заплакати під час вправлення, але після успішної репозиції біль швидко минає. Седація при типовому випадку не потрібна.

Знеболення за потреби: якщо звернення пізнє або діаґноз не цілком ясний — парацетамол 10–15 мг/кг per os кожні 6 год за потреби, максимум 60 мг/кг/добу. Ібупрофен 10 мг/кг per os кожні 6–8 год за потреби, максимум 30 мг/кг/добу; не застосовувати при дегідратації, нирковій недостатності, активній виразці/кровотечі ШКТ, гіперчутливості до НПЗП та у дітей до 3 місяців без локального протоколу.
Перша спроба

Гіперпронація

Посадіть дитину на коліна до одного з батьків. Однією рукою підтримуйте лікоть і пальпуйте головку променевої кістки великим пальцем. Іншою рукою утримуйте дистальне передпліччя або зап’ясток і плавно виконайте повну пронацію передпліччя. Іноді відчувається тихий “клік”.

Альтернатива

Супінація–флексія

Підтримуйте лікоть і пальпуйте головку променевої кістки. Повністю супінуйте передпліччя, після чого повністю зігніть лікоть одним контрольованим рухом. Метод доречний, якщо гіперпронація не дала ефекту.

Після вправлення

Оцінка функції

Через 10–15 хв дайте дитині іграшку, воду, наліпку або телефон вище рівня плеча. Після успішного вправлення біль зникає, а користування рукою зазвичай повертається протягом 5–30 хв.

Вправлення має бути плавним і контрольованим. Силовий потяг не потрібний і може лише посилити біль.

Якщо вправлення не вдалося

  • Зачекайте 10–15 хв і повторно оцініть функцію.
  • Повторіть одну спробу альтернативною технікою.
  • Якщо після двох спроб дитина не користується рукою — виконайте рентґеноґрафію та перегляньте діаґноз.
  • Залучіть ортопеда/педіятра при патологічному знімку, невпевненому діаґнозі або неможливості вправлення.
Ключова помилка: не повторювати болючі маніпуляції при набряклому, синюшному або локально болючому лікті. Таку ситуацію слід вести як можливий перелом, доки не доведено інше.

Післярепозиційне ведення

Після очевидно успішного вправлення гіпс, шина або пов’язка зазвичай не потрібні. Дитину можна виписати, коли вона спокійна і нормально користується рукою.

10–15 хв Менше болю, перші активні рухи. Спостерігати, заохотити гру й дотягування рукою.
5–30 хв У більшости гострих випадків рука функціонує нормально. Виписка з порадами для батьків.
Травма >12 год Функція може повертатися 1–2 доби. Косинкова пов’язка для комфорту; контроль через 48 год, якщо немає поліпшення.
Стійке невикористання руки Немає функції після спостереження або повторної техніки. Рентґеноґрама, диференціяція з переломом/вивихом/інфекцією, ескаляція.

Поради батькам

  • Не піднімати й не тягнути дитину за кисть, зап’ясток або передпліччя.
  • Піднімати дитину під пахви або за тулуб.
  • Негайно звернутися повторно при набряку, синцях, гарячці, посиленні болю, деформації або якщо дитина далі не користується рукою.

Для умов з обмеженими ресурсами

Якщо рентґеноґрафія недоступна, безпечна тактика — вправляти лише класичний випадок без набряку, деформації та локальної кісткової болючости. При будь-якій підозрі на перелом руку слід іммобілізувати й скерувати на знімок або до вищого рівня допомоги.
